1) For legal entities and enterprises with the status of an individual, carrying out entrepreneurial activity:

- Application for the issuance of the Residence Certificate (Form CCR17);

- Copy of the contract / document certifying the obtaining of income in / from the foreign state, with translation into the state language, except for contracts drawn up in Russian and English, for the tax period in which the residence certificate is requested.

2) For individuals citizens of the Republic of Moldova:

- Copy of the identity card or any other document certifying the identity and domicile of the individual;

- Copy of the contract / document certifying the obtaining of income in / from the foreign state or copy of the invitation to work, or confirmation issued by the enterprise in which the individual works, translated into the state language by authorized translators, according to Law no. 264- XVI of  December 11, 2008, except for contracts drawn up in English or Russian, for the tax period for which the residence certificate is requested;

- Copy of the extract from the register of shareholders and the extract from the minutes of the shareholders' meeting, and in the case of limited liability companies

- copy of the extract from the minutes of the founders' meeting, in case of receiving dividends.

3) For individuals foreign citizens or stateless persons who, for tax purposes, have obtained the status of a resident of the Republic of Moldova:

- Application for the issuance of the Residence Certificate (Form CCR17);

- Copy of the passport of the individual foreign citizen or of the stateless person's passport or the copy of the (provisional) residence permit or the copy of the identity card;

- The original document, issued by the Border Police, about the entries and exits in / from the Republic of Moldova for the respective fiscal year;

- Copy of the contract / document certifying the obtaining of income in / from the foreign state or copy of the invitation to work, or the confirmation issued by the enterprise in which the individual works. Translated into the state language by authorized translators, according to the manner established in Law no. 264-XVI of December 11, 2008, except for contracts drawn up in English or Russian, for the tax period for which the residence certificate is requested.

a) the document confirming the powers of attorney of the representative of the applicant / holder of the entrepreneurial patent - in case the application is submitted through a representative authorized by law;

b) the document confirming the payment of the patent fee, in an amount corresponding to the term of validity of the patent (for a period of not less than one month and not more than 12 months);

c) the documents confirming the payment of the state social insurance contribution for the entire requested period of activity based on the entrepreneur's patent or the documents confirming the exemption from paying the contribution (Record of the contribution transferred to the state social insurance budget by the holders of the entrepreneur’s patent, approved by Order of the President of the National Social Insurance House no. 461A of 13.10.2008). The documents confirming the exemption from paying the contribution, in the case of pensioners and disabled persons, constitute a copy of the pensioner or disabled persons card, and in the case of employees - the certificate confirming their insurance to the public social insurance system, issued by the employer;

d) the documents confirming the status of an insured person in the compulsory health insurance system or the documents confirming the exemption from paying the contribution (Certificates - model approved and issued by the Territorial Agencies of the NMIC);

e) in the case of the types of activity mentioned in headings: 1.1, 1.2, 2.7, 2.8, 2.9, 2.10, 2.11, 2.15, 2.16, 2.18, 2.19, 2.23, 2.24, 2.25, 2.26, 2.27, 2.28, 2.30, 2.40, 2.41 , 2.43, 2.46 from the annex to Law no. 93 - copy of the Notice on the commencement of trade and copy of the Notice of receipt of the notification;

f) copy of the diploma or other document regarding the previous studies and / or work activity, which confirms the level of qualification necessary to carry out the types of activity mentioned in positions: 2.1, 2.5, 2.6, 2.12, 2.13, 2.14, 2.15, 2.16 from the annex to Law no. 93;

g) two 3x4 photographs of the patent applicant.
